    Position:

    Social Worker / Case Manager

    Overview

    The full-time Case Manager, will provide direct social services (short-term case assistance and longer term case management) and recreational and health related activities to the mostly immigrant (Chinese) seniors who reside in a designated high need area of Manhattan's Chinatown. She or he will develop and coordinate a newly formed collaborative partnership between Confucius Plaza Housing Development, the Visiting Nurse Service of New York, and University Settlement to ensure that the services for seniors include supportive counseling, advocacy, assistance with concrete needs, long-term planning, crisis intervention, and planning and leading groups.  Since this is the launch of a new program, planning, program development, and community outreach will be a large part of the initial work. The goals of the collaborative project are to support seniors in the community to address immediate needs; connect to ongoing providers in a wide variety of service areas; and take recommended, evidence-based steps to prevent disease and increase wellness, enabling seniors to "age in place."

    Responsibilities:
    • Assist in education and outreach to the community (including residents, CBOs and other partners) on the new services.
    • Represent University Settlement and the NNORC on various coalitions and working groups aimed at increasing seniors' access to important medical, social service, and end of life information.
    • Develop and coordinate the collaborative working partnership between Confucius Plaza, Visiting Nurse Service of New York and University Settlement.
    • Coordinate the communication and all activities provided by our additional multiple partners who are providing services at Confucius plaza.
    • Perform intake, on-going assessment, and develop individualized, goal-oriented service plans to ensure that seniors remain healthy, in their homes, and receive the social, recreational, and health services they need.
    • Assist individuals in achieving goals through on-site and home-based support. Provide advocacy, information and referrals.
    • Work, in collaboration with the seniors and project partners, to identify the types of recreational and educational workshops would most benefit the Chinatown senior community. Organize and implement the groups and workshops. An understanding and ability to work within established cultural norms to help seniors best understand all available options is vital.
    • Link services to homebound and at-risk seniors to promote successful community living.
    • Provide crisis intervention and counseling as needed.
    • Coordinate and bridge services and case conferences and ensure there is no duplication of services between the three participating partners and to ensure seamless service delivery.
    • Document services provided to clients in writing (hard files and web-based case management system); keep client files up to date; provide regular statistical reporting on outcomes.
    • Coordinate with Project Home and other University Settlement staff for quality control and integration of services
    • Assist in designing documents or forms for record-keeping and outreach purposes; assist in program planning, evaluation and administration.
    • Perform other tasks consistent with this job description as assigned by the Director.
    Qualifications:
    • Bachelor's degree required (Master's degree preferred in Social Work, Counseling or related field)
    • Three years relevant experience required
    • Bi-lingual English/Cantonese required
    • Licensed Master Social Worker preferred
    • Willingness and credential to supervise social work students preferred
    • Excellent communication skills (verbal and written)
    • Excellent organization, communication, and follow-through a must as the position will work with three partners
    • Knowledge of social welfare/geriatric services systems in New York City, group work, and community outreach experience
    • Ability to work within a culturally diverse community and as part of a team
    • Tenacious advocacy skills and a commitment to "case to cause" direct services perspective
    Salary:

    Commensurate with experience and education, plus excellent benefits package

    Hours:

    35 hours/week, generally business hours but flexibility required
